1. Fill out the form in it's entirety.
• Be sure to have the following information available.
  • Oceanside School District
    145 Merle Avenue
    Oceanside, N. Y. 11572
  • Your personal information ie. Employee Id number, Verizon Wireless phone number, home and work phone numbers.
2. When complete, press the GENERATE FAX button.
3. A popup-window will display with the completed form. Be sure pop-up blockers are disabled.
4. Press the PRINT button to print the form.
5. Include a copy of your paystub/work ID badge with the form, and fax the form to 1-800-711-7788.

1. Enter your work e-mail and Cingular cell phone number
2. Click continue and then download & print a certification form from their website.
3. Follow the directions.

As a member of the NYS Retirement System you are a state employee and should use the following Foundation account number/ABI #045222 to secure your discount. Send a copy of your school ID, your Empire Card and your NYS Retirement card as proof. Delete any confidential information such as Social Security Number, direct deposit bank account numbers and salary details.
